GCR/Bridges Program

The GCR/Bridges Program is located at the St. Michael’s Health Centre location, 1400- 9th Avenue South.

What is Bridges?

Bridges is an alternative to institutionalized care where independence is maintained through ongoing monitoring for changes in functional status and the prevention of further limitations and injuries. It is our goal to improve quality of life for seniors by providing a program of health promotion in an inclusive environment.

This program offers restorative and rehabilitative services that facilitate a client’s ability to remain as active and independent as possible. It aims to support individuals in maintaining or improving their well being. It also assists in reducing caregiver stress.

How Bridges works:

A client may attend the program 1-5 days per week Monday through Friday. The program operates from 9:30am – 3:30pm on these days. Individual attendance may vary within the hours of operation to meet the need of the client, caregiver and the availability of services.

What is GCR?

Geriatric Community Rehabilitation (GCR) is a short-term outpatient program specializing in client assessment and rehabilitation. One of the goals of GCR is to support the frail elderly and their families in their desire to remain at home and in the community.

How GCR works

The program involves a health care team approach that works closely with the client, family physician and community agencies such as Home Care. Attendance is scheduled for two days per week. Clients can choose morning or afternoon programs dependant on availability of space.
